Job Description
Job Overview:

Responsible for managing site specific stroke program encompassing service excellence, compliance, and communication. ;
Work with Medical Director and stroke manager on the site-specific stroke program.;
Works with service line administration and medical leadership at the hospital site in the development, modification, and adherence to stroke clinical pathways, standard order sets and best practices. Assists in the development and systematic dissemination of applicable educational materials to distribute and use in caregiver, patient, and family educational efforts.;
Works as a liaison with ancillary departments to provide effective communication in achieving stroke program goals. ;
Effectively communicate with and report to Stroke Program manager regarding process, program status or issues regarding duties routinely or as needed.;
Effectively uses data to determine areas for and impact of improvement activities in stroke clinical, operations and financial arenas at site level. ;
Monitor overall care of the stroke patient. Will respond to site stroke alerts in ED and Inpatient areas while on hospital campus and assist physicians and staff as patient condition warrants. ;
Monitor the care given to stroke patients through chart review, patient/family interview and on-site visitation;
Continually evaluate facility process to ensure compliance with program standards;
Collaboration with physicians and staff to promote care delivery that ensures efficiency, cost effectiveness and expected clinical outcomes.;
Collaborate with Stroke Medical Director, Stroke Program manager, service line leadership, interact with physicians, hospital staff and administrators to help define and participate in process improvement activities to attain program goals and meet external requirements.;
Coordinates multidisciplinary process improvement activities associated with the Stroke Program to advance clinical effectiveness. This will require both collaborating with TH Process Improvement Specialists and leading multidisciplinary teams through a standardized problem-solving methodology and /or mentoring. ;
Collaborate with the Medical Director and stroke program manager to facilitate the achievement and maintenance of clinical targets and goals for the stroke program.;
Effectively coordinating the stroke program maintaining compliance with any external regulatory requirements.;
Coordinate with data analysts and abstractors across the organization to provide and ensure clinical knowledge and process perspectives to meet agreed upon reporting requirements.;
Collaborate with research coordinator to ensure clinical trials are related to the care and treatment of stroke patients at hospital site. ;
Improve the quality of care and movement of patients through the initial acute care phase of their hospital stay in a timely fashion.;
Track relevant clinical metrics and benchmark data and initiate projects as warranted to decrease stroke length of stay and stroke mortality.;
Monitor and assure the process surrounding the system stroke program meet or exceed the requirements of the system designated regulatory agencies.;
Education- staff, patient, EMS, community, others (As needed) within the realm of stroke systems of care.;
On-call as needed;
Other duties as assigned.

American Heart / American Stroke Membership
5-7 years experience Clinical Nursing

Job Responsibilities:

Ensure maintenance of stroke certification standards for Bethesda North.
Works with prehosptial partners in providing stroke education and works to grow stroke program network by engaging EMS agencies and hospitals outside of TriHealth. Participate in Cincinnati Stroke Consortium and other community events as required by TJC stroke certifications in TriHealth Hospitals.
Monitor stroke metrics for Bethesda North Hospital; Proficiency in AHA/ASA Get with the Guidelines stroke registry; Completes Certification Measure Information Process (CMIP) quarterly and annually for CSTK, STK measures and PI projects; action plan on constant improvement for applicable stroke metrics. Contributes to biannual survey visits and intracycle phone calls for PSC/TSC certification. Develops opening and data presentations for biannual site visit for stroke certification.
Collaborates with department and unit leadership and educators to ensure relevant and recent evidence-based education is provided to providers and care givers. Ensure monthly stroke meetings occur with stroke coordinators at BNH including case reviews. Ensure appropriate participants attend meetings as required by TJC. Collaborates with team to ensure that stroke patient education is relevant to patient population and unit-specfic to level of care provided by staff.
Run monthly site stroke steering committee meeting with input from stroke program manager and medical director. Provide direction for Bethesda North to ensure growth of stroke program as part of the Neuroscience Serviceline. Ensure attendance of appropirate participants as required by TJC. Performs 90 day follow up calls to all interventional (thrombectomy, alteplase) stroke patients for Modified Rankin Scale (per TJC standard).
Participate in Neuroscience Committee meetings in collaboration with stroke medical director, Neuroscience chief, Neuroscience NPs, and other appropriate clinical staff. Aware of NIH and sponsored research trials in which TriHealth is participating.
Excellent organizational skills including ability to work independently and set priorities. Maintains professional knowledge and skills through nursing practice, in-service, and continuing/formal education. Obtains contact hours annually to maintain licensure and 8 hours of CEU as a member of the stroke core team annually. Participates in regional and national conferences surrounding neuroscience-related topics as determined by stroke program manager and/or physician director. Returns from conferences and provides education to staff on topics covered.
