Job Overview:

The purpose of this position is to serve as part of the ambulatory treatment team providing behavioral health consultation and direct service to patients and families. Duties include consultation, assessment, brief individual/family/group counseling/psychotherapy, education, prevention, medical exam room-based intervention, referral and service coordination with other community agencies and programs. Assist with program development and evaluation as directed. Perform other clinical, administrative, educational and/or research duties (commensurate with training and ethical scope of practice) as needed and directed.

Job Requirements:

Master's Degree
Licensed Independent Social Worker; (LISW)
Licensed Independent Social Worker or (LPCC)
Licensed Professional Clinical Consultant
Generalist knowledge of a wide variety behavioral health conditions and evidence based treatment modalities to serve diverse groups of patients across the lifespan
Must be able to work in a team based integrated brief treatment model
Up to 1 year experience Professional Clinical Social Worker or Clinical Consultant

Job Responsibilities:

Provides behavioral health services including consultation, brief assessment, and evidence-based treatment in an ambulatory care medical setting, including brief clinical interview, administration and scoring of standardized assessment tools, clinical diagnosis of behavioral health conditions, psychotherapy/counseling and supporting documentation. Deploys evidence-based interventions such as motivational interviewing, problem focused therapy, mindfulness, cognitive behavioral therapy, and acceptance and commitment therapy. Assists patients in developing skills to facilitate disease self-management.
Develops and maintains a collaborative relationships with the primary care physicians, ambulatory practice team, other behavioral health team members, patients and families to include, providing behavioral health education to staff, patients and families in individual and group settings. Works collaboratively with community mental health providers and community support services to develop, provide and support a continuum of care for patients.
Works collaboratively with the primary care team to provide referral, case management and follow up services for patients with behavioral health needs to include, engaging patients, initiating referrals, addressing barriers to care and follow-up. Employee should be familiar with community resources to address patient psychosocial and behavioral health needs.
Provides accurate and timely documentation in Epic, Agility or other authorized electronic health records to support the identification and treatment of patient's psychosocial and/or behavioral health needs. Provides assistance in capturing program evaluation and fidelity measures as directed.
Works collaboratively to assist in monitoring the behavioral health program and identifying areas for improvement. Participates in meetings and quality improvement activities, continuing education and serve as a member of committees, as requested.

Other Job-Related Information:

Incumbent must be an independently licensed clinical social worker or Licensed Professional Clinical Consultant and experienced in providing consultation, assessment and treatment of behavioral health problems for patients across the life span and willing to work in a fast paced team based practice.
